Carpal tunnel syndrome
Carpal tunnel syndrome is one of the most common compression injuries of the nerves. It develops when the median nerve is squeezed in the narrow canal at the wrist, leading to numbness, tingling and weakness of the hand. At our neurology practice in Bratislava we reliably confirm the diagnosis with an EMG examination and set the next steps.

What carpal tunnel syndrome is
The carpal tunnel is a narrow canal on the palm side of the wrist through which the median nerve passes together with the tendons. When the space in the tunnel narrows or its contents enlarge, the nerve is compressed and its function gradually worsens. This is known as a compression neuropathy.
The median nerve supplies the thumb, index finger, middle finger and part of the ring finger, which is why the problems appear precisely in these fingers. The condition most often develops with one-sided overuse of the hands, but also in diabetes, thyroid disease or during pregnancy.
Symptoms of carpal tunnel
Problems typically start at night
- Numbness and tingling of the thumb, index and middle finger, especially at night and towards morning
- An urge to "get the hand moving" or shake it out so that the problems subside
- Worsening during the day with activities involving a bent wrist – phone calls, driving, working with a mouse
- Reduced sensation in the fingers and an uncertain grip, dropping small objects
- In the advanced stage, wasting of the muscles at the base of the thumb and permanent weakness of the hand
Diagnosis – EMG is the gold standard for confirmation
We begin with the initial neurological examination – a 30-minute consultation with a medical history and clinical tests of sensation and strength of the hand. The clinical picture can point to carpal tunnel, but an objective measurement is needed to confirm it unequivocally and determine its severity.
The gold standard is the EMG examination, which precisely measures the conduction of the median nerve across the wrist and determines how severely the nerve is compressed. It is EMG that reliably tells carpal tunnel apart from other causes of numbness – for example problems originating in the cervical spine or polyneuropathy.
When not to postpone the examination
- Permanent numbness of the fingers that no longer subsides after shaking the hand out
- Visible wasting of the muscles of the palm at the base of the thumb
- Noticeable weakness of the grip and dropping objects from the hand
- Rapid worsening of the numbness over a few weeks
Frequently asked questions about carpal tunnel
The gold standard is the EMG examination, which objectively measures the conduction of the median nerve across the wrist and determines the severity of the compression. Clinical tests point to the diagnosis, but EMG confirms it.
No. The surgery is performed by orthopedics. We confirm the diagnosis with EMG, determine its severity and, if surgery is needed, prepare the documentation and direct you to a suitable facility.
No. You book directly by phone and you will usually get an appointment within a few days, with no referral.
Many of us sleep with a bent wrist at night, which increases the pressure in the carpal tunnel. That is why the numbness typically appears towards morning and eases after shaking the hand out.
